Anti-Human Tissue Transglutaminase (anti-tTG) antibodies are autoantibodies directed against tissue transglutaminase, an enzyme that modifies gluten-derived peptides in the small intestine. These antibodies are a highly sensitive and specific biomarker for celiac disease (CD), an autoimmune enteropathy triggered by gluten ingestion in genetically predisposed individuals. Anti-tTG antibodies mediate an immune response that damages the intestinal villi, leading to malabsorption, nutrient deficiencies, and gastrointestinal and extra-intestinal symptoms. Detection of anti-tTG, particularly of the IgA isotype, is central to diagnosing CD and monitoring response to a gluten-free diet.

In the clinical setting, anti-tTG IgA is the preferred serologic test for celiac disease screening and diagnosis due to its high sensitivity and specificity. It is especially useful in patients presenting with malabsorption, chronic diarrhea, anemia, or other signs of gluten intolerance. For patients with selective IgA deficiency, anti-tTG IgG testing is recommended to avoid false-negative results.

In research, anti-tTG antibodies serve as a biomarker for studying gluten-triggered autoimmunity, intestinal inflammation, and mucosal immune responses. They are used in epidemiological studies to identify undiagnosed celiac disease, track disease prevalence, and evaluate early immune responses to gluten.
